Home-Based Casework
Home-Based Casework is an intensive, in-home service designed to help families meet Department of Child Services goals while building essential life and parenting skills. Our team supports families through parenting skills coaching, home safety and organization, budgeting and time management, communication and conflict resolution and accessing housing, jobs and more.

How it Works
We begin after receiving a referral from DCS. We meet the family and create a service plan. Then we create a schedule of consistent, supportive, hands-on casework. We monitor outcomes and provide documentation. With goals met, we wrap with a final report and resource plan.
Open to Referrals
We accept hard-to-place and urgent referrals. Our team is ready to service all clients. Referrals can be sent in the portal using our familytieshaven@yahoo.com email address.
